Although insurance policies issued to Trust did not contain beneficiary provisions that Trust requested, the policies nevertheless were binding and premium was owed where the policies were delivered to Trust’s insurance broker whose constructive knowledge of the policies’ terms, plus payment of the premium, constituted acceptance by the Trust.
